Procedures & Next Steps

  1. Schools should assess for the need for emergency services.  If a student reports an active plan with intent to self harm, the student should be referred to the Emergency Room for psychiatric screening.  If unsure, please contact us to discuss.  However, please note that we do not have a 24-hour answering service and cannot be relied upon for emergency consultation. 
  2. The referral source is responsible for obtaining consent from the legal guardian to refer to Skylands Wellness.   
  3. The guardian must consent to Skylands Wellness sharing any information with the school and/or district.  This consent is part of the informed consent for clearance. 
  4. A preliminary clearance letter can be provided at the time of assessment for both service level options.  For comprehensive risk assessments, please allow 1 week for completion of the full report and biopsychosocial assessment. However, any urgent concerns will be discussed with the family and school at the time of assessment.  
** Upon completion of the referral form, Skylands Wellness will receive e-mail notification and begin to outreach to the school and parent.  Please allow 2 hours during school hours before reaching out to us.  If you have not heard from us within 2 hours, call 973-527-4539 or e-mail info@skylandswellness.com.  Referrals made after 2 pm on Friday will be addressed on Monday.
